Large and small: a telltale sign of acute pontomesencephalic injury.

چکیده
An 85-year-old woman was found comatose. Her right pupil was 2 mm and reactive while the left pupil was 6.5 mm and nonreactive (figure, A). Oculocephalic and corneal reflexes were absent, the cough reflex was preserved, and there was spontaneous bilateral decerebrate posturing. Traditional teaching indicates that midbrain lesions cause fixed, midposition pupils while pontine lesions cause pinpoint pupils. Rarely are these findings present simultaneously in the same patient.1,2 One midposition and one pinpoint pupil in a comatose patient indicates a combined lesion of the midbrain and pons, caused in this case by an acute occlusion of the basilar artery.

Acute effect of burn trauma to small skin areas on myocardial ischemia reperfusion
Introduction: It has been reported that traumas such as transverse abdominal incision before myocardial ischemia result in a significantly decreased infarct size. This phenomenon is named remote preconditioning of trauma. Since small skin burn is one of most common traumas, the effect of this injury on ischemia-induced arrhythmias and infarct size was investigated in a rat model of ischemia-...
متن کاملPrecedence of Parenchymal Enhancement on CT Angiography to a Fatal Duret Hemorrhage
We report a case of fatal duret hemorrhage (DH) in a patient with acute tentorial subdural hematoma and bilateral chronic subdural hematoma along the cerebral hemispheres. Preoperative CT angiography (CTA) revealed prominent parenchymal enhancement in the ventral pontomesencephalic area. After burr-hole drainage, a large hemorrhage developed in this area. The parenchymal enhancement in the CTA ...
متن کاملAcute sleep deprivation preconditions the heart against ischemia/ reperfusion injury: the role of central GABA-A receptors
Objective(s): Central γ-aminobutyric acid (GABA) neurotransmission modulates cardiovascular functions and sleep. Acute sleep deprivation (ASD) affects functions of various body organs via different mechanisms. Here, we evaluated the effect of ASD on cardiac ischemia/reperfusion injury (IRI), and studied the role of GABA-A receptor inhibition in central nucleus of amygdala (CeA) by assessing nit...
متن کاملCo-Incidence of Acute Appendicitis and Appendiceal Transection after Blunt Abdominal Trauma: A Case Report
A 13-year-old boy with a history of bicycle handlebar injury to the left side of his abdomen was brought to the Emergency Department of our center. On admission, his vital signs were stable and abdominal examination revealed ecchymosis and tenderness of the injured areas. Mild to moderate free fluid and two small foci of free air in the anterior aspect of the abdomen, in favor of pneumoperitone...
متن کاملEffect of Intravenous Injection of Erythropoietinon Hospitalization Period in Patients with Acute Spinal Cord Trauma
Background & Aim: Spinal cord injury (SCI) has a very long history, and its cases form a large proportion of patients admitted to trauma centers in Iran. Studies show that repair after spinal cord injury can be done. In fact, many pharmaceutical agents, such as erythropoietin (EPO), are applied to reduce secondary injury following the initial disorder and to maintain the nerve tissue. Methods &...
